The Tailor Institute has a contract with The Department of Mental Health (DMH).

 

Consumers must meet the following requirements:

A disability, eligible for SSI and Medicaid; must be initiated prior to age 22.

Case Management is done through Catholic Charities of Southern Missouri (CCSOMO) or equivalent.

 

A consumer receives evaluation through CC; qualification; service plan is written based on weaknesses, selects provider.  

The service coordinator develops ISP (individualized service plan) with input from consumer and TTI Staff.
